I noticed this moment during my rewatch of some of the earlier episodes last night and I think I finally realised why Dean always seems to sleep with his arm outstretched to Sam. I think Dean started doing it because Sam would reach for him in the night if he had a nightmare when they were kids. The habit never stopped, because Sam never stopped reaching for him, just like he did during this episode.
